I've investigated some more and here's what I found... Though I was not able to reproduce the exact same bug as you, I did notice on my PC that if I resized the pop-up window in Intellicontact it eventually lost the status bar at the bottom. If I then let go of my mouse, the window is resiezed for good, without a status bar, so I can't make it bigger again. Now in my case, I can just close the window and when I want to insert a picture again, it re-opens with it's initial size - not the minimized one.
So I disable the Monitus.net extension, and sure enough I can resize pop-ups and not loose the status bar. I then installed another extension that has a very similar interface to the MNE (Zotero) and it did the same thing as our extension! So to me this says it might be a FireFox issue.
Of course that doesn't help you much. ITry this: go in "Tools - Options - Content" and click on "Advanced..." Then check the "move or resize existing windows" and then "OK". Try to add a picture in IC again - does it come back to the right size again?
In the meantime, I'll try and find out more about this resizing issue.

I trid to reproduc this bug over here and couldn't. Can you please send me a picture of one pop-up that's not displayed right? Sometimes a picture is worth a thousand words...
